At our residential care facility, we provide tasty, nutritious, and well-balanced meals served three times daily. Our meal schedule is designed to allow residents to enjoy their meals at a leisurely pace, and we also offer healthy between-meal snacks. Special diets and dietary restrictions are carefully accommodated to meet individual needs.
Safety Features
Our residents’ safety is our top priority. Our facility is designed with thoughtful features to ensure a secure environment for all residents. Hallways are furnished with sturdy handrails for added support and stability. Continued Personal Care
We understand that attending medical appointments and therapy sessions can be challenging, which is why we strive to bring necessary services directly to our residents whenever possible.

- Medical supplies (available at an additional cost)
- Physical/ Occupational/ Speech Therapy as directed by a physician.
Please note that hospice care, home health services, medical supplies, and all therapies are provided by licensed outside organizations.

I had been told my dad wouldn't live past 3 months. He was coherent and said he was ready for a residential home and by sheer fate or luck one of his nurses recommended TO HOME CARE and that there was an opening at 451 Columbia I quickly called said please hold and went to see the next day . l was taken a back at how beautiful the place was and the staff seemed to care and I said yes. My father under the incredible Care and Love of everyone but especially Bernard and Jenny & Janey they succeeded to bring back into him a healthiness he hadn't had for so long that he stayed with us 2 years and 2 months....Home made meals and the strong friendship he built with Bernard and his hospice care team from Buena Vista Hospice was why he was here so long.Bernard became his best friend during this time able to spend quality time , listen to his stories, watch movies record his tales, dirty jokes and poems. When it came to him passing last week I was there a couple hours before it happened when I left Bernard and Jenny comforted him and shared their love that it was ok to let go. I was called 2 hours after I left and he had just passed peacefully unaided by any medical help. Bernard called me and we were all crying and laughing at the person he was . My dad left a mark on everyone and Bernard and Jenny & all left a memorable magical mark on our entire family while my dad was in their care at Thousand Oaks Home Care 451 Columbia.Sincerely,Dana Flores
Jenny and Bernard are the most caring and compassionate caregivers. My mom had taken a bad fall and needed extra care. When my Dad and I first met Jenny and Bernard we knew Mom would be in great hands. They treat their clients like their own family members. Dad and I would visit daily and we were always greeted with a warm hello. The level of care is excellent. The home is always clean, with home cooked meals and baked goods. I loved that they have a routine of getting up , showered and dressed daily. They all ate at the kitchen table just like any family would. Any time I needed to take Mom to the dr they were there to assist if needed. I was always given a current list of medicine and current vitals to have in hand to answer any questions from the dr. Honestly Jenny and Bernard became our family and we are beyond blessed by them. My Dad eventually needed some extra help while undergoing treatments and Jenny and Bernard welcomed him in with open arms and got him back on his feet. It truly gave me peace of mind knowing both my Mom and Dad were in an excellent loving ,kind, compassionate home that was a mile from my home. Words will never be enough , I could never say Thank you enough for all the care, love and support Jenny and Bernard gave to my parents, to me, and my entire family . They are a true blessing.
When I walked into this home care residence as a possible place for my mother, I knew I got lucky. I had done extensive research on places such as this one, and visited about 4 or 5 actual residences prior to choosing Thousand Oaks Home Care. The care here is outstanding! You know how you can walk in to a home care facility and there's a certain unclean, unfresh odor? Well, not here! TO Home Care is bright, always clean, and the residents are always well-kempt and presentable all day, every day. Jenny and Bernard cook all three meals daily, serving a well-balanced menu full of variety. The back yard is so pleasant as well. Comfortable seating, lots of flowers and fruit trees and beautiful birds eating away at the nectar. My mom lived there for three years before her passing. She was a diabetic, receiving insulin shots twice a day from a visiting nurse. Jenny was like having an extra eye, making sure all my mom's nurse appointments were kept and helping manage my mom's medications with the utmost care precision. When I think of Jenny and Bernard, I think of softness, kindness, care and compassion. I agree with the other comments about this facility. It is an exceptional home for your loved one!
Bernard and Jenny were absolutely wonderful to my mom when she was staying there at Thousand Oaks Home Care. She was bedridden after having had a stroke and they were extremely caring and helpful to her. The home was always very clean, bright and pleasant.
